When a child is faced with a serious, chronic illness, Tri-State Lifecare (TLC)’s Home Pediatric Palliative Medical Care services can help bring comfort and improved quality of life for patients and their families while still at home.

Our highly qualified palliative team cares for patients infant age and older who have been diagnosed with:

• Heart & Lung Conditions

When enrolled in the Home Pediatric Palliative Medical Care program, a TLC provider will come into your home to provide support as needed. The TLC team act as an extension of your primary care/specialty provider, assisting with medication adjustments, discussing illness symptoms and identifying needs within in the home to help improve quality of life. It’s like the doctor’s office coming to you and your child instead of having to travel to the doctor.

Patients enrolled in the In-Home Palliative Care program may continue to receive services like home health care, chemotherapy and other curative treatments.
